thank you for joining us we're gonna go over our new patient portal so first navigate to our website
and click patient portal
fill in the necessary information then click next then we're going to click get verification code then we're going to check our email for the code
and we're gonna copy and paste the code
in this window we can read through these terms of use and privacy policy as well then we're going to click verify and log in
so we do need an email on file to use the portal so we can get that six digit verification code that code cannot be text only email and we are on a desktop for this video we have two other videos one for an android and one for an iphone but this is the home screen of our new patient portal so we can kind of scroll through here and see the different information we can find
on forms we can complete any necessary forms so the forms don't show up well on iphones and android mobile devices so if you do need help filling these forms out we can help you in the office but they seem to work better on desktop computers
when we're first filling out the medical history form it might be easier to mark everything as null and then fill in any yeses later mine are already selected because i filled this form out before so once we fill this form out once any previous responses will be pre-populated
so we'll click next if we're done
same thing for allergies
and then we can go through these questions
and then if you're done you can click sign form or you can sign in the office whichever you prefer
on this tab we can see any upcoming or past treatments
any payment histories
and upcoming or past appointments
if you have an appointment that's not confirmed there will be a button here where you can confirm we kindly ask that you don't come from your appointment too far in advance maybe a week or sooner
if we go down to account details this is where we can change our address email emergency contact phone number if we click the box next to show insurance details
you can verify the insurance we have on file for you
we can also see the information for our family members by toggling here
now this is my son's information his treatments his payments his appointments and any forms he needs to complete and then his home screen
back to my account for the forms uh we're going to stop doing the kobit 19 screening forms we now have a new form it'll be called the apcdc update form just to verify your information on insurance and we do have some code questions on there as well as some other information that might be helpful if your form is not here like in my case the update form is not here please give us a call we can send it from the office into your portal once you do complete a form a new one will automatically be placed in here 14 days later for you to fill out for your next appointment mine's not here because i just did my form in another video but if you have any questions please give us a call and again we do have ipad stations set up where we can help you fill out the forms especially for the first time in the office thank you.
Video #2 for Patient Portal on Ipad
let's navigate the patient portal on an iphone so we'll go to google chrome go to our website ialprocitydentalcare.com then we'll click on patient portal
fill in the information
then we're gonna get the verification code
so we'll navigate to our email application by pressing the home screen
refreshing and then opening the email if needed copying the six digit code and going back into chrome
and pasting the code so for an iphone i think you need to double tap the home screen to toggle between different applications
and now we're in the home screen of the portal
so again we can see our forms upcoming and past treatment payment histories and we can update the account details as well our address and email and phone number can all be updated here
for both ourselves and our family members and at the very top if you click show insurance details you'll be able to see what we have on file please let us know if there are any changes and then we can select other members of our family so we're going to jump into my son's account now so we can check his balances upcoming appointments previous appointments
treatments and any of his forms that need to be completed
so please complete the update form before your visits and then at least glance through your medical form to see if there are any changes previous answers will be pre-populated again sorry about the formatting these forms don't seem to carry over well to mobile devices
so i can't zoom in or out even flipping the screen doesn't help
but if you have any questions or need help we do have ipads available in the office and we can help you complete these forms
and just signing the form again

Video #3 for Patient Portal on Android
on an android device we'll navigate to our website and click patient portal
we'll fill in the necessary information
then we'll click get verification code then we'll navigate to our email application by pressing the home button on your device
your email if needed by dragging down on the screen
then we're going to copy the six digit code and navigate back to google chrome our website by clicking the three bars on the lower left then we're gonna paste the code
and click verify and log in
this is the home screen of our patient portal on an android device as our balance upcoming appointments and any pending forms
so for this video we want to go over the apcdc update form this is another way to navigate to the form by clicking the forms on the three lines
and then clicking the apcdc update form once you fill in this form your previous answers will already be populated
please answer the questions to the best of your ability
this helps us ensure accurate and proper payment from your insurance
these questions will replace our existing cobit form
and you can navigate through the other information on this form
once you're done you can click sign form
if you need help and would prefer to finish in the office click save as draft
and just quickly i wanted to scroll through the medical history form
these answers will also be uh pre-populated with previous um responses if you scroll down to the very bottom you can mark everything as no and then go back up and change anything to yes that might be a little easier
and you can either save as draft to sign in the office or click next these forms seem to work better on a desktop some of the formatting isn't quite carrying over onto either the iphone or the android
we're back at the home screen and i'm just clicking through different areas of our patient portal so you can see your upcoming or your past dental treatments
you can see your payment history
and any upcoming or previous appointments
you can also confirm your appointment via your portal
but please do so maybe a week or so before your appointment please don't confirm your appointment too far in advance
on the account details section you can change um your email and address phone number and at the very top if you click show insurance details you can see what insurance we have on file again please call us if there are any changes in your insurance once you're done you can click sign out
